Application Information

If you desire to practice barbering in Nevada, you may pick up an application at the office listed above for $15.00 or you may send $15.00 plus $2.00 postage and an application will be mailed to you. This deposit is not refundable.

General Information

Please make money orders payable to the Nevada State Barbers’ Health and Sanitation Board.

You must make arrangements to furnish your own living model for the part of the examination demonstrating your work at the barber chair. Registered Barbers, Apprentice Barbers, Barber Students, Cosmetologists, or Instructors will not be accepted as models. Model Standards must be followed.

Models must have sufficient facial hair on all parts of the face and neck (side burns, cheeks, lip and neck); to demonstrate all phases of a facial shave. The minimum hair length, top and crown area must be at least 3 ½ inches in length, on curly hair models, models must have at least 2 inches in length. You may only use 1 model. All demonstrations must be on the same model. If your model is disqualified the day of the examination, you will forfeit all money. You may bring your model by any board office to have him inspected before the date of the examination.

Your completed application must be in the office of the secretary by the 15th of the month preceding the examination. Anything late will be scheduled for the next examination.

You will be notified of the location, date and time to appear for your examination after the deadline for that particular examination. Ex: If date of exam is December, you will be notified after November 15th.

You are required to show up 15 minutes before your examination time, if the examination has started and the doors are locked, you will not be able to take any part of the examination and your money will be forfeited.

Examinations are given the first part of March, June, September, and December; and this completed application must be in the hands of the Secretary of the Nevada State Barbers’ Health and Sanitation Board before the 15th day of the month preceding the examination date.

This is a professional examination, and you are expected to dress like a professional.

Before turning in your application, make sure you go over the general information check list.

If you are not a citizen of the United States of America, please submit a copy of your resident card, green card etc. and social security card to prove that you are entitled to work in this country.

CHECKLISTS

Before mailing the application to the SECRETARY of the Nevada State Barbers’ Health and Sanitation Board, please check to see that you have completed the following requirements:

APPRENTICE EXAMINATION:

• Have you completed the front page? Name, address, phone number etc.?
• Have you included a copy of your high school diploma or GED?
• Have you included a copy of your Barber School diploma?
• Have you included your graduation letter from a recognized Barber School, indicating completion of the required number of hours of instruction?
• Have you included two (2) passport photographs (2 x 2) of yourself? (Polaroid or home photos are not accepted)
• Have you completed the declaration on page 3? Is it notarized?
• Have you completed your completed health certificate on back page? Is it notarized?
• Have you included any balance due to the Nevada State Barbers’ Health and Sanitation Board?

REGISTERED EXAMINATION:

• Have you completed the front page? Name, address, phone number etc.?
• Have you included a copy of proof of at least a 10th grade education?
• Have you completed two (2) affidavits on page 2 (at the bottom) showing at least 5 years work experience in the last 10 years? Are they notarized?
• Have you completed the declaration on page 3? Is it notarized?
• Have you completed your health certificate on back page? Is it notarized?
• Have you included two (2) passport photographs (2 x 2) of yourself? (Polaroid or home photos are not accepted)
• Have you included any balance due to the Nevada State Barber’s Health and Sanitation Board?
• Have you included a copy of your current paid up barber license from the State in which you are coming from?

COSMETOLOGIST CROSSOVER:

• Have you completed the front page? Name, address, phone number etc.
• Have you included a copy of proof of at least a 10th grade education?
• Have you completed the declaration on page 3? Is it notarized?
• Have you completed your health certificate on back page? Is it notarized?
• Have you included two (2) passport photographs (2 x 2) of yourself? (Polaroid or home photos are not accepted)
• Have you included any balance due to the Nevada State Barber’s Health and Sanitation Board?
• Have you included a copy of your Barber School diploma?
• Have you included your graduation letter from a recognized Barber School, indicating completion of the required number of hours of instruction?
• Have you included a copy of your current paid up Cosmetology License from the State of Nevada?
